Providing access to e-Urząd Skarbowy of the company allows you to promptly respond to requests from tax authorities, as well as to submit NIP-8, PCC-3, and other tax declarations, forms, and powers of attorney while minimizing your involvement.

To fill out the electronic version, follow the link and fill in the fields according to the screenshots below. If the option to fill out the form electronically is unavailable, it can be downloaded, printed, and filled out manually according to the sample.

In field 1 of the application, it is necessary to indicate the NIP of the filing entity (Company).
In section A, enter the full name of your tax authority (Urząd Skarbowy).

In section B, mark point B.1 (Granting access).


You can check the exact name of your tax authority here by the postal code of the registered office address (siedziba spółki).

Section C.1. Data of your organization
  • In point 2, enter the NIP of your organization.
  • In point 3, indicate the full name of the organization according to the KRS.
  • In points 4–8, enter the registered office address (siedziba spółki): street, house and apartment numbers, postal code, and city.
  • In point 9, provide a contact phone number if necessary.
Section C.2. Type of access provided
Mark the Podstawowy (primary) box, as the Rozszerzony (extended) option is only required for specific cases.

Section E. Data and signatures of the persons representing the organization
In this section, those who grant access (the company's management) sign.
  • Fields Nazwisko and Imię: Surname and first name of the board member according to the KRS.
  • Field Stanowisko / Funkcja: Position (for example: PREZES ZARZĄDU or CZŁONEK ZARZĄDU).
Field Podpis:
  • For electronic submission: Leave this field blank. There is no need to write or draw anything in the application itself, as the entire document is signed virtually using Profil Zaufany (or a qualified signature) directly before submission in the system. This action is fully sufficient to give the application legal force.
  • For paper submission: If you still decide to bring the application in person to the tax office, a legible signature (podpis czytelny) is placed here. In Poland, this is a strict rule: you must fully write your First Name and Surname by hand, letter for letter as in the official document (passport or residence card).
Congratulations, all data in the form is filled! All that remains is to sign the application and send it to the authority.

How to send electronically:
  1. Download the completed application by selecting the DRUKUJ button (keyboard shortcu Ctrl + P / ⌘ + P), choosing the "Save as PDF" option in the "Printer" field".
  2. Sign the finished form with your Profil Zaufany.
  3. Send the file via the e-Doręczenia system, as well as via e-mail to your manager.
If the form was filled out manually:
  • You can scan it and sign it electronically via Profil Zaufany in the same way for online submission.
  • If the form is both filled out and signed physically "on paper," it must be sent by registered mail or submitted to the tax authority in person.

Instructions for sending via e-Doręczenia

The screenshot below shows the process of creating an official letter to the tax office. The application must be sent not from the organization's account, but from the personal account of the natural person in whose name the application is being submitted. To ensure the document is accepted correctly, follow these steps:

To (Do): Enter the name of your Urząd Skarbowy. You can find the e-Doręczenia address of your office in the search engine.
When filling out the fields in the e-Doręczenia message, you must specify:
Temat wiadomości (Message subject) – Przyznanie dostępu w e-Urzędzie Skarbowym
Treść wiadomości (Message content) – Wniosek o przyznanie dostępu do konta organizacji w e-Urzędzie Skarbowym
In the Załączniki (Attachments) block, attach your completed application that has been signed with a profil zaufany.
After filling in all the fields and adding the completed form as an attachment, select Wyślij (Send) and send the letter. The sent letter will appear in the section Moja skrzynka → Wysłane.
